In each of our clinics we have designated an individual responsible for the protection of personal information. These individuals are:

If there is reason to believe that an incident involving a breach of confidentiality has occurred, the designated person must: investigate the circumstances of the incident, identify the scope of personal information affected as well as the person(s) and the cause of the problem.

If the incident is minor and causes no consequence:

• The person responsible will ensure that any risk of future harm is diminished and will undertake action to avoid a recurrence of the situation
• The person responsible will record the privacy issue in the register provided for this purpose
• The person responsible will review the procedure(s) in place and ensure its application

If the incident demonstrates a risk of serious harm being caused:

• The person responsible will communicate with the Commission d’accès à l’information du Québec
• The person responsible will notify the person(s) concerned (unless the notification is likely to hinder an investigation according to article 63.8 paragraph 3 of the “Loi de l’accès à l’information” (LAI))
• The person responsible, in a discretionary manner, can communicate the situation to the Board of Management for their consideration so that necessary precautions can be initiated in other clinics and to avoid similar incidences
• The person responsible will record the incident in the register provided for this purpose
